In my office a few years back, the chair of OB was proposing a ratio of nocturnalists to midwives to cover his service. In many of these conversations, I have to try and interpret unfamiliar vocabulary (really jargon) without making visible my ignorance. Nocturnalists, I learned, were a type of hospitalist that delivers babies during off hours to minimize the stress on the staff. Got that. The argot still made me uneasy.

It was the noun “midwife.” Boy, did that feel archaic. It harkened back to Beowulf (in a bad way). Do I go to a urologist and a “semihusband” performs a procedure on my prostate? I wonder, with our international readership, what the translation of “midwife” is in other languages? It does seem like a word which is crying out for an update.

My brother-in-law is an ocular plastic surgeon. “Plastic” surgeon seems like another of those terms which is past its prime. It reminds me of the movie “The Graduate” (50+ years ago), where a minor character states, “There’s a great future in plastics. Think about it.” Perhaps that is why practitioners now often use “cosmetic,” or to cover all bases, “cosmetic and reconstructive” surgery. The latter is too long a set of proper nouns to sublimate into the vernacular.

My favorite out of date medical proper noun is one near and dear to my heart – “Radiologist.” Radiology refers to x-rays (or other high energy radiation). MRI is not radiology by that precise definition. Which is why when our authors state “three observers assessed radiologic images using ….”, JMRI corrects that to just “images.” Radiologic is not only redundant, but incorrect.

Images leads us to “imager.” Now, as commonly used, imager means anyone (usually a physician) who assesses images. Imager would not be a good replacement for the incorrect noun of Radiologist – too nonspecific. Maybe “imagehusband,” or have two groups – “imagers” and “semi-imagers” (needlessly disparaging to rads like me who only interpret a portion of the imaging spectrum [pun intended]), or perhaps Diagnostic Anatomists. Still imperfect, as the latter ignores physiologic imaging and especially AI – so they would still be “demi-imagers.”

Maybe Diagnostician sounds best. Since the full name of the field is Diagnostic Radiology, this is not much of a stretch. When I come home after a day reading MSK films from the EDs around Detroit, the positive feedback of making clinically useful diagnoses all day long is invigorating – and I usually need some invigoration. Although all “imagers” do is make diagnoses, I suspect (and to some degree I empathize) “clinicians” (another incorrect proper noun) would be quite miffed by this renaming suggestion. A clinician might say “you interpret, but we conclude.” A semantic conundrum begging for a diagnosis.
